This survey was initially created by the Connecticut Library Association's Americans with Disabilities Act (ADA) Committee in April 2006.

It is designed to be answered by Connecticut public and academic library directors or those staff designated to deal with ADA and library accessibility issues.

The survey is designed to create a database of libraries that own adaptive technology equipment and can be used as a tool to help other libraries make decisions about adaptive technology equipment they are considering. Adaptive, or assitive, technology is any item, piece of equipment, or product system, whether acquired commercially, modified, or customized, that is used to increase, maintain, or improve the functional capabilities of individuals with disabilities. (Assistive Technology Act, 1998).

We estimate this survey will take approximately 20 minutes to complete.
